Elevate Healthcare accepts HSA/FSA and files with all insurance companies if we are not in network we offer single case agreements with in network rate. If you wish to receive services at Elevate Healthcare as an out of network hospital, then a single case agreement (SCA) may be required. A single case agreement (SCA) is an agreement between Elevate Healthcare and a non-contracted insurance or other coverage plan. The SCA outlines authorization and payment terms prior to services performed. A patient can also request a single case agreement (SCA) directly from his/her insurance or other coverage plan if they want to have services at Elevate Healthcare. The insurance or other coverage plan would work directly with Elevate Healthcare to complete the SCA prior to services performed.

Be sure to write down Date, Time Called, Representative Name, and Reference number for the call. As the parent/guardian you are your kids advocate and can assist to expedite an authorization especially if there's no provider within 20 miles radius of your home. Client should not have to travel outside of reasonable distance for care or family participation according to the mental health parity act.

All Co-Pay, deductibles, and Co-Insurance is due upon admission and time of services rendered. A valid card must be on file to process balances due upon each visit.

The amount that clients pay is determined by their health insurance coverage. This price list does not apply to patients with some form of insurance, including Governmental coverage such as Medicare or Medicaid. Pricing is for specific services listed, and does not include complicating factors or services provided by independent physicians.
All of our services are considered mental health services and are covered by insurances. We are contracted with most major healthcare insurance plans. Rates for appointments and programming are dictated by your insurance plan coverage for mental health and not by Elevate Healthcare. Please contact your insurance company your insurance company prior to your scheduled appointment to verify your copay, deductible, co insurance so you are prepared and aware of your financial responsibility before you are seen. We suggest when you speak with your insurance representative be sure to ask for a reference number of the call. In the event you do not have insurance, we do offer set self-pay (non-insurance) rates for services below.
